You need to have it out to make use of it, and you need to be within reach of the target. It is a good Implement to have with the Weapon to have a little extra Temp HP. It's only use is healing and Temp HP.They Exploit Vulnerability and learn it has weakness 20 to good damage. But it also has resistance 20 to all physical damage that isn't cold iron. Thaumaturge doesn't have a cold iron weapon so they just use a regular metal weapon. The attack hits. So then with damage, as per step three of the damage rules, no immunities apply.

You may wield a single, one handed weapon, (this includes wielding a single free hand weapon) and combine that with your esoterica, your Implements, and even manipulate your scrolls (scroll Thauma), all with your 'spare hand'. This is far beyond what the base rules allow for the average hero.
